Making a referral

We would ask that you visit and assess us prior to making any offer of a formal referral. We are however happy to take telephone referrals, followed up formally in writing.

In response to a formal referral to our services, a day assessment will be conducted. This follows a standard format which seeks to fully evaluate the individuals’ current level of functioning and subsequent health and care needs.

The visit will be supported by a full and detailed report which seeks to summarise the process of the assessment and make recommendations as to the suitability of the individual to our service and vice versa.

The assessment will be carried out by one or both members of the senior team and is free of charge. – make free of charge bolder
